Adult Waiver and Release
In consideration of being allowed to participate in any softball-related activity organized, hosted, sponsored, administered, or supervised by Softball 619, including but not limited to practices, games, scrimmages, clinics, camps, tournaments, workouts, warm-ups, travel related to participation, and use of fields, dugouts, batting cages, parking areas, and other facilities, I acknowledge and agree as follows:
1. Participation in softball activities is voluntary. I choose to participate with full knowledge that softball and related activities involve inherent and other risks.
2. I understand that participation in softball may involve risks including, but not limited to, collisions with players or objects, being struck by bats, balls, or other equipment, falls, sliding, overexertion, uneven playing surfaces, weather-related conditions, transportation-related risks, and other risks that may result in serious bodily injury, illness, permanent disability, death, and damage to personal property. I knowingly and voluntarily assume all such risks for myself.
3. To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue Softball 619, its officers, directors, managers, employees, coaches, assistant coaches, volunteers, officials, umpires, agents, representatives, sponsors, affiliated organizations, event hosts, facility operators, property owners, lessors, and any other persons or entities acting in any capacity on their behalf from and against any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to participation in the above activities, including claims based on ordinary negligence.
4. This waiver is not intended to release claims that cannot be waived under applicable law, including claims arising from gross negligence, reckless conduct, or intentional misconduct.
5. I agree to follow all posted rules, safety requirements, league rules, and reasonable instructions from coaches, officials, staff, and volunteers.
6. I certify that I am physically able and sufficiently fit to participate in softball activities and that I have disclosed any condition that may affect safe participation.
7. In the event of injury, illness, or emergency, I authorize Softball 619 and those acting on its behalf to obtain or arrange emergency medical care for me if needed. I accept responsibility for any medical expenses incurred.
8. I understand that Softball 619 does not guarantee or provide health, medical, disability, or accident insurance for participants unless expressly stated in writing.
9. I grant permission to Softball 619 to photograph, record, and use images, audio, or video of me for lawful promotional, social media, website, archival, or informational purposes without compensation.
10. This agreement shall be binding upon me and my heirs, assigns, personal representatives, next of kin, and estate.
11. I have carefully read this waiver and release, understand its contents, understand that I am giving up substantial legal rights, and agree to it freely and voluntarily. I intend my electronic or handwritten signature to be legally binding.
I am 18 years of age or older, I have read and understand the waiver and release above, and I agree to be bound by it.
Minor Participant Section
Minor Participant Waiver and Release
In consideration of my minor child or ward being allowed to participate in any softball-related activity organized, hosted, sponsored, administered, or supervised by Softball 619, including but not limited to practices, games, scrimmages, clinics, camps, tournaments, workouts, warm-ups, travel related to participation, and use of fields, dugouts, batting cages, parking areas, and other facilities, I acknowledge and agree as follows:
1. Participation in softball activities is voluntary. I choose to allow the minor participant to participate with full knowledge that softball and related activities involve inherent and other risks.
2. I understand that participation in softball may involve risks including, but not limited to, collisions with players or objects, being struck by bats, balls, or other equipment, falls, sliding, overexertion, uneven playing surfaces, weather-related conditions, transportation-related risks, and other risks that may result in serious bodily injury, illness, permanent disability, death, and damage to personal property. I knowingly and voluntarily assume all such risks on behalf of the minor participant and myself arising from the minor participant’s participation.
3. To the fullest extent permitted by law, I, individually and on behalf of the minor participant, release, waive, discharge, and covenant not to sue Softball 619, its officers, directors, managers, employees, coaches, assistant coaches, volunteers, officials, umpires, agents, representatives, sponsors, affiliated organizations, event hosts, facility operators, property owners, lessors, and any other persons or entities acting in any capacity on their behalf from and against any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to the minor participant’s participation in the above activities, including claims based on ordinary negligence.

11. I represent that I am the parent or legal guardian of the minor participant, that I have authority to sign this agreement on the minor participant's behalf, that I have carefully read this waiver and release, understand its contents, understand that I am giving up substantial legal rights, and agree to it freely and voluntarily. I intend my electronic or handwritten signature to be legally binding.
